ScoreSentry is a configurable intake, scoring, readiness, and risk workflow platform operated by RioPlex Technology LLC.

Decision-support only

ScoreSentry helps collect, organize, score, and route structured information. It does not provide legal, medical, emergency, clinical, financial, or professional advice and does not replace human review.

Provider responsibility

Providers are responsible for their questionnaires, scoring logic, thresholds, notices, result language, use of results, and compliance obligations.

Demo portal

The demo portal is provided for evaluation and may contain sample workflows, test data, or demonstration configurations.

Acceptable use

Users may not attempt to misuse the platform, bypass access controls, interfere with service operation, or submit unlawful, harmful, or unauthorized content.
